Insurances

I am an in-network provider and will file claims for the following insurance providers:

• Medicare

• Lyra

• Cigna

• United

• Optum

• Ambetter

• Aetna

Although I do my best to verify insurance prior to your appointment, if your insurance does not pay you will be responsible for the balance.

*I do not accept EAP services other than Lyra. I accept Traditional Medicare but not most Advantage plans.

* All fees are due at the time of service. You may pay by credit card, check, cash, or health savings account (HSA)

* If you are not able to make your appointment please cancel 24 hours in advance to avoid a late cancellation/ no show fee.
